Lots of Monday morning quarterbacking going on today. One prevalent rumour is someone having been directed to load something over limit. Not sure of the details, but if that ends up being the case, it will be one of the first that I know of clearly indicating a lack of control or scrutiny over what happens during these builds.
Pointed out to a friend today how many setups I've worked in proximity of over the years that saw little to no observation or enforcement from anyone other than those building them, which had people in harms way. Rarely does anything bad happen, but rarely isn't enough. Second or third party involvement many have to be the case for everything that happens in this industry, starting ASAP. Yes, it will complicate things for many people, but if it saves people from having to experience these type of horrors, it will be worth it.
