Uploading video... What format?

So, since I see so much video out of sync with audio in places like YouTube, I'm curious if there's a consensus as to what is the best format to use to avoid these kind of issues. I'm not talking about sync at the production stage, I'm talking about stuff that is easy to tell happened during the transfer to some digital format somewhere along the way. What is it that is usually the culprit, assuming there is a usually, and what can you do to avoid it from happening? Does whatever works for YouTube also keep you safe with any other sites or transfers?

Best bet is as follows

Video
H264 codec. Whether in an .mp4 wrapper or .mov shouldn't really matter.
24, 25, or 30 FPS
Any standard resolution in either 4:3 or 16:9 aspect ratio should work fine but make sure you use a square pixel, or 1:1 pixel aspect ratio.
Progressive scan
Audio

AAC encoding
320kpbs
44.1kHz
Identical file length to video.

These are the specs for YouTube and Vimeo. Not sure about other sites but I'm guessing they're similar. There are lots of other options and formats that you can use, but I stick to these and have basically zero issues.
